A few possible reasons: some are marked "fixed" when the patch in
another bug actually fixes them. Sometimes people don't click the
"patch" box. Sometimes people upload their patch as a zip, and so can't
- because currently things marked as patches are served as text/plain (I
think.)

Not if the procedure of the developers involved means that the patch is
not necessarily attached to the bug.
