Every rule has an exception, too :-) Occasionally, a bug will be
- FIXED without a patch because the solution was trivial
- FIXED without a patch because the module it touches doesn't
require peer review (I think Phoenix/Firebird went through this
phase, and maybe for many changes it still does)
- FIXED without a patch because somebody selected the wrong
Resolution (it should have been WORKSFORME, for instance).
The main point is that the module defines the review process (and not
all modules require review) *and* that this is a human process, and as
such, people make mistakes, forget, ignore policy, etc.

Because not all attachments are necessarily patches?
