New custom fields schema

Hi folks--

I've updated my custom fields schema, incorporating several changes
based on recent discussions.  I've also fixed a few unrelated errors in the
original document.  Oh, and I've added some foreign key constraints, which
are purely for documentation purposes, since MySQL doesn't enforce them
AFAIK.
