Revised custom fields requirements & schema

Hi folks--

I've revised my requirements document in response to discussions here.  I've
also taken a first stab at a custom-fields-supporting database schema which
will be the core of a forthcoming design document.

I couldn't find any mention of whether attachments are allowed on the
developers' list, but a random scan of the archives *seemed* to suggest that
they're OK, so I'm attaching the two files to this message.  If the message
if rejected, or if people yell at me to stop, I'll resend them as two regular
messages.
