5.0.4 branch created

I have created a 5.0.4 branch in the GitHub repository. 

It is forked from the 5.0 branch immediately prior to the perltidy commit (which was the first commit that shouldn't have been on that branch). All of the changes included in 5.0.5 other than the perltidy were prior to the perltidy so we automatically got those. 

There were a few documentation changes and one code change (viewport meta) in 5.0.6 and one code change after 5.0.6 that hasn't been in a release yet (MIME header issues) which I cherry-picked onto the new branch. The main change in 5.0.6 was the table format changes in the database (which also shouldn't have been on that branch). 

You can see the new changelog for the 5.0.4 branch at [ https://github.com/bugzilla/bugzilla/commits/5.0.4 | https://github.com/bugzilla/bugzilla/commits/5.0.4 ] and this will become the upcoming 5.0.4.1 release, pending release notes and other final release activities.
