Funky dependency mails

There's a problem that's been around since shortly after we branched for
2.16 (the problem doesn't exist in 2.16, but only on the tip), where mail
about a dependency state change will get doubled intermittently (it doesn't
happen all the time, either).  We *cannot* ship Bugzilla 2.18 with this
still happening.  Unfortunately, nobody has a clue what's causing it, and
the code surrounding it is not obvious.

If there's anyone out there that can donate some time into trying to track
this down, it would be really, really appreciated!

It's been widely speculated that the big massive mail rewrite on bug 84876
will probably fix this by virtue of rewriting that chunk of code, but it's
obvious at this point that 84876 isn't going to make it for 2.18, so we've
no choice but to find and fix this bugger in the existing code :)
