Mathew,
In my previous employer we ended up building such an in-house
bug-tracking/release management system. We essentially integrated our
version control tool (CVS at the time) with Bugzilla, and wrote extra
scripts that would report bugs that went into a release (I did not
participate in this -- someone else did this work).
Since I left that employer I started writing from scratch this
integration (with a different design and some extra goodies) with the
ultimate goal of bulding such a release management system. I desperately
needed this integration. You may be interested in the work I have

This was recently discussed in the Bugzilla mailing list, as I am
currently pushing for a universal tool integrating source code version
control with bug tracking.
