Bug lifetime schema

When we implemented Bugzilla here we created the following document
(attached) to show the lifecycle of a bug.  This is (I think) slightly
more complete than Tomas' diagram - it includes a couple of additional
loops that were not mentioned, however it has some references to certain
specific job titles we have here (and it's in MS powerpoint format).
