Custom fields design doc

Hi folks--

Attached is my custom fields design document, or at least what I've
completed of it so far.  It cuts off abruptly in the middle of describing
editcustomfields.cgi, but I think enough of the basic groundwork has been
laid to make it worth posting for comments.  If you have any, I'd appreciate
hearing 'em.  Thanks.
