The important part (to me) is that the code that comes out of this 
discussion will be the best ideas of quite a bright folks.
 
With all the comments, I'm not quite sure if there's enough consensus 
to hack on a patch tonight, so I'll hold off.  Should there be a new 
bug created for my stylesheet/header patch?

I think my first order of business is to discern what structure we will 
use for the pages.  In an informal test (IE6 and Moz 1.5) <h4> looks to 
be 'normal' font size, but bold.  Anything else is smaller than the 
normal text, which is obviously not what we want.

Perhaps the version text (Bugzilla 2.17.6) could be an <h4> or <h5>,  
the explanatory header (bugzilla bug X) can be an <h2>, and then we 
have <h3> for any remaining header-type stuff.  The 3 horizonal 
divisions of the page header can stay <td>'s.
