After 2.16.3 upgrate - can not add new users.

I upgraded from 2.16.2 to 2.16.3 and everything seemed to work,
until I tried to add a new user. Not being a perl expert I just do
not see anything obvious when looking at gobals.pl or createaccount.cgi .

The apache logs show:
